$20.00 per person per module plus $75/hour for training/prep/system/testing input time (approx. 3 hours for up to 5 people)

In coordination with MSHA Representative to conduct test.  

This test is conducted by an MSHA representative and is presently set for the third Tuesday of April and October.  

  (Students must present proof of prior MSHA training/qualification through MSHA white card for prior year.

 2-3 days based on number of students.  As need basis contact for more info.

6-7 Hours based on the specific requirements

$75 daily per dosimeter; $150 daily for instruction, installation, removal and reports.   If 4 or less dosimeters are used, then $100 per dosimeter; $200 daily for instruction, installation, removal and reports.  If out of 35 mile radius, per Diem will apply. 

$75 daily per dust sampling unit; $200 daily for instruction, installation, removal and reports.  If out of 35 mile radius, per Diem will apply. 

Conducted at main location in a certified sound booth by CAOHC certified technician.  Will include evaluation form from medical professional.  Call for pricing.

$100 per standard 30 min presentation.  Extra cost apply if handouts are requested.  If out of 35 mile radius, per Diem will apply.

$850 initial (excluding employee training input) plus $60 per plan,   $800 annual maintenance if customer has up to 10 ISN clients, 860 for 11-15 clients, call for quote if > 15 clients.  $75 / hr for help with client RFIs, tracking and inputting employee training.
